Diabetes Care

Ongoing management of type 1 and type 2 diabetes — blood sugar, medication, diet and the yearly checks that prevent complications.

Diabetes is a condition we manage over years, not in a single visit. Whether you were diagnosed recently or have lived with it for a while, our goal is the same: keep your blood sugar in a healthy range and stay ahead of the complications diabetes can cause.

What we monitor

  • HbA1c and your daily blood sugar readings
  • Blood pressure and cholesterol
  • Kidney function and urine protein
  • Foot and eye checks, with referral when needed
  • Weight, diet and medication review

Type 1 and type 2

We care for adults with type 2 diabetes and work alongside your endocrinologist or diabetes educator when you have type 1. If you use insulin or a continuous glucose monitor, bring your readings and we will review them together.

What to bring

Bring your glucose meter or phone app, a current list of your medications, and any lab results from the last few months. If you have not had an HbA1c recently, we can order one at your visit.
