Bring to your first visit
- Photo ID and your insurance card
- A current list of every medication and supplement you take
- Records or results from prior doctors, if you have them
- Your questions — write them down beforehand so you do not forget

Call us and we will send them to you, or pick up a printed copy at the front desk. Please do not email completed forms or medical records.
