Forms we use
Completing these forms before your visit shortens check-in. Call us and we will send them to you, or pick up a printed copy at the front desk. Please do not email completed forms or medical records.
- New patient registration — your contact details and insurance information.
- Medical history — your past conditions, surgeries, medications and allergies.
- Privacy practices acknowledgment — confirms that you received the Notice of Privacy Practices for your practice.
- Consent to treatment and financial responsibility — lets us treat you and explains billing.
- Medical records release — only if you want us to request records from another doctor.
What to bring
- Photo ID and your insurance card
- A current list of every medication and supplement you take
- Records or results from prior doctors, if you have them
